Step 1: Create Your Account

You will be getting an email with the subject line “[CHL] Dr. Kelly has invited you to Practice Better” Open the email and click the link to create your HIPAA secure account.

This is where Dr. Kelly and her team will be communicating to you, sending forms, secure chat access and secure video calls.

Step 2: Fill out paperwork

There will be 3 forms for you to fill out. These must be filled out in order for us to get moving on our side.

2 of the forms are signature forms and the 3rd form is where you give us all your current and past health information.

Step 3: Dr. Kelly will make additional testing recommendations

Dr. Kelly will personally analyze and create testing recommendations based on the Master Health Asset List that you have filled out.

We will reach out to you with the tests that Dr. Kelly recommends. If you choose to add in any additional tests then we will send over an invoice for those and once paid we will send all the kits and submit your order to LabCorp.

If you choose not to add any additional tests, we will send everything out to you and submit your order to LabCorp.
